UNF Sponsored Activity
Prior to participation in any University of North Florida (UNF) sponsored Experiential Learning activity that occurs not at UNF, the participant must complete an Acknowledgement form:

Student Acknowledgement
For when students are engaging in an off-site internship, practicum, or Experiential Learning opportunity as part of an experience. Use this form if: (a) the student is regularly off-campus for the experience, or (b) a faculty/staff member is NOT present at the location for the experience.

ACKNOWLEDGMENT
(General Release for Emergency Medical Treatment, Student-Provided Transportation & FERPA Release)
(Off-Site Internship, Practicum, Transformational Learning Opportunity, etc.)

Field Trip Release for an Off Campus Activity
For any course field trip or when a course will be meeting at a location off the University campus. Use this form if: (a) the student is NOT regularly off-campus for the experience (1-2 occasions), or (b) a faculty/staff member is present at the location for the experience.

ACKNOWLEDGEMENT, INDEMNITY, WAIVER and RELEASE OF LIABILITY FOR PARTICIPATION in a UNIVERSITY OF NORTH FLORIDA FIELD TRIP OR OTHER OFF CAMPUS EVENT

Note that certain activities and locations might require students to complete more forms or acknowledgements prior to participation.

Non-Sponsored Activity
The University of North Florida built Engage UNF on the GivePulse platform. GivePulse has partnerships with non-profit organizations and community agencies that also use its platform. These organizations can affiliate with the University of North Florida and share their events and activities on the Engage UNF platform.

The University of North Florida does NOT sponsor these events and activities. For any activity not sponsored by UNF and found on Engage UNF, it is the responsibility of the participant to do their own due diligence prior to participation. The University of North Florida is not responsible for harm, loss, damage, or injury associated with such activities.
